Machine learning-based detection and area quantification of mechanical and corrosion damage in femoral stem tapers

Corrosion and mechanical damage at modular femoral taper junctions remain important degradation mechanisms in total hip arthroplasty. Yet, their assessment commonly relies on ordinal visual scoring methods that reduce spatially heterogeneous damage to a single severity grade. Here, we present a SegFormer-based semantic segmentation framework for pixel-level identification and quantitative assessment of tribocorrosion damage on Ti6Al4V tapered shafts following controlled hip simulator testing. In brief

Researchers built a SegFormer-based semantic segmentation system to label corrosion and mechanical damage pixel by pixel on Ti6Al4V tapered shafts after hip simulator testing. It was trained on 576 manually annotated high-resolution scanning electron microscopy images, and five independently seeded models were combined for ensemble inference.

The approach matters because damage at modular hip tapers is usually judged with a single ordinal grade that hides how uneven the damage is. Area and region-size measurements could give implant researchers a richer picture of degradation. The evidence is a proof of concept on two representative taper cases from simulator testing, not a demonstration in patients.

Main points

  1. On the independent test set the final model reached an IoU of 0.678 for corrosion and 0.496 for mechanical damage, with overall pixel accuracy of 0.947.
  2. Training used a hybrid weighted cross-entropy and Dice loss and a three-stage hyperparameter optimization strategy.
  3. Ensemble analysis found more corrosion area, and more spatially concentrated corrosion, in the Ti6Al4V/CoCrMo case than in the Ti6Al4V/Ti6Al4V case.
  4. One 0.50 to 1.00 mm2 region accounted for about 23.6% of the corrosion area in the Ti6Al4V/CoCrMo case.

The gain

A trained segmentation model gives pixel-level, full-surface measurement of corrosion and mechanical damage on femoral stem tapers, replacing single-grade visual scoring and cutting manual effort on large SEM datasets.

The rundown

The dataset images were annotated into background, corrosion, and mechanical damage classes, using SEM and energy-dispersive X-ray spectroscopy analyses plus literature to identify damage features.

The two demonstration cases paired Ti6Al4V shafts with either Ti6Al4V or CoCrMo femoral heads, and the ensemble was used to assess variability between seeds in the predictions.

Region-size analysis showed that larger localized regions contributed substantially to total corrosion area, a distribution detail that a single severity grade cannot capture.
