The ethics-driven data governance framework for health-based AI (EDG-AI4H): an integrated approach for empowering explainable AI in health insurance risk prediction in Iran

Purpose: The digital transformation of health data has enabled predictive models in insurance. In Iran, the absence of an indigenous data governance framework integrating ethical, legal, and technical considerations poses serious risks to responsible AI development. This study aimed to design an ethics-driven data governance framework to empower Explainable AI (XAI) in health insurance risk prediction in Iran. In brief

Researchers in Iran developed a data governance framework called EDG-AI4H to support explainable AI in health insurance risk prediction. They combined a narrative review of international articles and Iranian policy documents with interviews of five experts, analyzed thematically in 2024.

The framework matters because Iran lacked a homegrown approach joining ethical, legal, and technical concerns, which the authors say poses serious risks to responsible AI development. Whether it works in practice is unknown, since it has not been piloted and rests on input from a small expert group.

Main points

  1. The framework has four hierarchical, interacting layers: Governance-Ethics, Operational-Integration, Intelligence-Analytics, and Value-Decision Making.
  2. Governance-Ethics covers data ownership policies, dynamic consent management, a Data Ethics Board, and legal compliance.
  3. Intelligence-Analytics requires XAI-by-design, active bias monitoring, and model lifecycle governance.
  4. Value-Decision Making includes preventive and personalized insurance and policyholder transparency and empowerment.

The gain

A four-layer governance framework with mandatory explainability and bias monitoring offers a roadmap for transparent, trustworthy AI risk prediction in Iran's health insurance sector.

The rundown

The qualitative study used Braun and Clarke's six-step thematic analysis in MAXQDA 2022. Data came from a narrative review and semi-structured interviews with experts in insurance IT, health data science, IT law, epidemiology, and insurance management.

The framework was validated through a focus group with the same five experts, and the study followed COREQ reporting guidelines. The Operational-Integration layer adds a metadata-driven catalog, standardized data exchange, data quality assurance, and secure storage architecture.

The authors conclude the framework links ethics, technology, and business and could potentially reduce ethical and legal risks while fostering public trust.
